Job Summary

The Registered Nurse (RN) is a professional nurse responsible for assisting with planning and directing nursing care for a specific number of residents.

This position directly supervises charge nurses and assists with teaching and supervision of other team members in the provision of quality care consistent with the mission of Wesley Towers.

Performance Requirements

Personnel Responsibilities

  1. Provide positive leadership and supervision of nursing personnel.
  2. Assist with the orientation and training of newly hired personnel.
  3. Report employee absences, late arrivals and inappropriate actions to the charge nurse and Unit Care Coordinator, and assist in ensuring substitute staff is called.
  4. Participate in ongoing teaching of staff by modeling professionalism, consistent quality care and initiative, and by taking opportunities for informal teaching.
  5. Assist with making assignments for team members.
  6. Supervise passing of medications by CMAs to residents in the assigned area.
  7. Evaluate completion of tasks assigned to ancillary personnel.
  8. Participate in evaluation and corrective action of personnel in conjunction with the charge nurse, Director of Nursing and Unit Care Coordinator.
  9. Notify the charge nurse or Unit Care Coordinator of problems that arise during the shift.
  10. Propose and execute corrective actions as needed.
  11. Evaluate care provided to residents by team members.
  12. Be able to cope with mental and emotional stress related to this position.
  13. Be able to function independently and demonstrate flexibility, personal integrity and consistency.

Coordination of Resident Care

  1. Assist in meeting and maintaining established standards of nursing care for residents in the assigned area.
  2. Assist in the ongoing development, updating and execution of the resident plan of care.
  3. Assist with resident assessments and documentation according to facility policies.
  4. Advocate for residents and their needs.
  5. Provide nursing interventions in a safe and caring manner.
  6. Interpret the established resident care plan and any changes to members of the team.
  7. Coordinate changes in physician orders with the resident's plan of care.
  8. Assist physicians when making rounds and/or examining residents.
  9. Observe, assess and report changes in resident behavior or physical condition as necessary to the physician, family, designated contact person and Unit Care Coordinator.
  10. Administer medications and treatments according to physician orders.
  11. Assist with discharge planning and teaching on admission, ongoing and at discharge.
  12. Receive, document, execute and follow up on physician orders and complete appropriate paperwork or computer work according to policies and procedures.
  13. Delegate appropriate tasks to other members of the nursing team consistent with their job descriptions, certification and state licensure.
  14. Supervise and evaluate quality completion of delegated tasks.
  15. Maintain communication with resident families and legal health care representatives regarding resident condition, needs and changes in the plan of care.
  16. Communicate resident needs and changes to the nursing coordinator and during shift report.
  17. Serve as a positive role model to staff, residents and families at all times.
  18. Communicate positively and effectively to maintain an atmosphere of warmth, personal interest and positive focus, with the holistic well-being of the resident as the focal point.
  19. Assist with planning and implementing appropriate therapeutic individual and small-group activities to fulfill residents' needs and interests.
  20. Execute and direct nursing care in accordance with regulations, guidelines, standards and Wesley Towers policy.
  21. Perform duties of CNA, CMA or other nursing staff as needed to fulfill the resident's individualized plan of care.
  22. Provide residents with opportunities for independence and choice consistent with their care plan.
  23. Perform duties within the nurse's scope of practice that help residents attain or maintain the highest possible level of functioning.

General Duties

  1. Interpret the individualized plan of care for each resident to other members of the team.
  2. Supervise CNAs and CMAs in providing quality, personalized care for each resident in the assigned area.
  3. Together with a CMA or another professional nurse, complete counts of restricted drugs as necessary.
  4. Complete admission nursing assessments of residents, transfer sheets and discharge instruction sheets for residents leaving the facility.
  5. Complete discharge summaries upon discharge of residents.
  6. Coordinate with other team members, departments, unit coordinators and physicians to ensure successful transfer of residents.
  7. Provide incoming team members with a report of pertinent information regarding residents assigned to the team.
  8. Together with the Unit Care Coordinator, ensure the resident's physician is notified of changes in condition and that the physician's plan of care is carried out.
  9. Attend Care Planning sessions for assigned residents when possible.
  10. Complete applicable documentation on resident charts, including resident responses and outcomes related to the plan of care.
  11. Initiate appropriate action as resident needs arise.
  12. Work with and cooperate with facility consultants and implement changes as authorized by the Director of Nursing.
  13. Ensure all procedures for administration, storage, ordering, disposal and control of medications and supplies are followed by personnel.
  14. Demonstrate safety and cost effectiveness in the delivery of care according to standards, policies, procedures and regulations.
  15. Practice universal precautions and proper disposal of infectious waste at all times.
  16. Perform TB testing as needed for residents.
  17. Perform TB testing for team members as directed by the Home Health Coordinator.
  18. Report incidents, accidents and exposures according to policy.
  19. Assist in identifying safety hazards and emergency situations and initiate immediate action as needed.
  20. Respond to emergency calls throughout the Wesley Towers complex as assigned.
  21. Assist residents with transfers, promote ambulation and exercise, and transport residents as needed and as designated on the resident plan of care.
  22. Use gait belts, proper transfer techniques and lifts according to facility policy and procedure.
  23. Use and operate equipment in a safe manner.
  24. Report unsafe or defective equipment.
  25. Follow all safety precautions.
  26. Note safety hazards and report or correct them immediately.
  27. Assist in the nursing process as requested by the Director of Nursing and Unit Care Coordinator and according to State of Kansas licensure.
  28. Assist in the continuous service improvement process as requested by the Director of Nursing, Unit Care Coordinator or CSI committee.
  29. Be aware of the location and contents of all standards, policy and procedure manuals and safety data sheets, and know how to use them.
  30. Be familiar with all regulations governing nursing and resident care and know where those regulations are located.
  31. Cooperate with all personnel within and between departments, customers and outside contacts in a team-like atmosphere.

Qualifications

  1. Currently licensed as an RN in the State of Kansas.
  2. Ability to read, write, speak and understand the English language.
  3. Capable of understanding, following and/or issuing oral and written instructions.
  4. Ability to communicate with other licensed nurses and accept counsel from the Unit Care Coordinator.
  5. Ability to develop positive rapport with other staff, residents, families and other departments.
  6. Positive and understanding attitude toward residents with dementia and elderly or ill residents.
  7. Ability to establish and maintain effective working relationships within the department and with other departments throughout the facility.

Special Demands

Must be flexible as schedules change.

The employee must understand that errors or incompetence can cause serious consequences for residents and must demonstrate patience and tact while communicating effectively with residents, families and staff members.

Must have good eyesight to observe residents, provide nursing care, read thermometers and complete documentation.

Must be able to hear normal and soft tones to hear resident calls.

Employees should be aware that working with residents with dementia may involve exposure to residents who become violent, combative or aggressive without notice or provocation. Staff members must respond appropriately while remaining calm and non-threatening.

Must be able to distinguish smells and be familiar with and capable of operating special equipment used for the benefit of residents within health care.

Working Environment

  1. Works in a well-lit and ventilated building.
  2. May be exposed to residents with communicable diseases.
  3. May be exposed to disagreeable odors.
  4. Possible exposure to sharp materials and/or instruments.
  5. May be subjected to stressful situations and uncomfortable surroundings.

Physical Demands

  1. Frequent sitting, standing, walking, bending, squatting and reaching above the head.
  2. Must be able to crawl in case of fire.
  3. Must be able to lift up to 30 pounds unassisted.
  4. Must be able to grasp and perform fine manipulation with both hands.
  5. Must be able to push and pull carts.
  6. Must be able to push residents in wheelchairs.
