PrairieStar Health Center recognized for excellence in heart health, diabetes care, and quality outcomes

PrairieStar Health Center has once again earned national recognition from the U.S. Health Resources and Services Administration (HRSA), receiving multiple Community Health Quality Recognition awards for excellence in heart health, diabetes care, access to care, preventive health, quality outcomes, and health information technology.

The awards place PrairieStar among top-performing community health centers nationwide and celebrate the organization’s ongoing commitment to providing high-quality, patient-centered care for the communities it serves.

PrairieStar was named a National Quality Leader in Heart Health, a highly competitive recognition awarded to health centers that meet or exceed national benchmarks for cardiovascular disease prevention and management.

Sonya Schwein-Finecy, PrairieStar’s Quality Improvement Coordinator, said the recognition is especially meaningful because heart disease remains the leading cause of death in the United States. She emphasized that preventive care plays a vital role in reducing the risk of heart disease, stroke, and other serious health conditions.

“This recognition celebrates PrairieStar’s continued commitment to advancing cardiovascular health and delivering high-quality preventive care,” Schwein-Finecy said.

PrairieStar also received National Quality Leader recognition in Diabetes Health, celebrating performance that exceeds national benchmarks in clinical quality, patient outcomes, access to diabetes care, and disease management.

In addition, PrairieStar earned recognition for Preventive Health, Improving Health Care Access, and Advancing Health Information Technology for Quality, highlighting the organization’s success in preventing illness, expanding access to care, and using technology to support quality improvement and patient care.

“We place a strong emphasis on quality and innovation across our Information Technology department,” said Mandy Zipprich, Chief Operations Officer. “Our team works diligently to maintain secure, reliable systems while continually improving the experience for providers and patients. This recognition celebrates their commitment to excellence and the important role technology plays in supporting high-quality care.”

HRSA’s Community Health Quality Recognition (CHQR) program recognizes health centers that achieve quality improvements in access, clinical quality, health outcomes, and health information technology using the most recent Uniform Data System (UDS) data.

“These awards are a direct reflection of the dedication, skill, and compassion of our staff,” said Bryant Anderson, Chief Executive Officer of PrairieStar Health Center. “They demonstrate that we are not only meeting the highest national standards for quality care but also making a meaningful difference in the lives of the patients we serve.
