Resident Care Unit Coordinator (LPN) – Assisted Living

The Wesley | Hutchinson, KS
Day Shift | Full Time
Salary Range: $28–$41 per hour

The Wesley is seeking a Resident Care Unit Coordinator (LPN or RN) to serve Collier Assisted Living and Hester Living Center.

Position Summary

The Care Coordinator is a licensed nurse responsible for carrying out resident care plans and assisting the Assisted Living Administrator in the overall operations of Collier Assisted Living and Hester Living Center.

This role ensures residents’ physical and mental well-being are maintained at the highest practicable level while upholding state and federal regulations and The Wesley’s standards of care.

Reports To: Assisted Living Administrator FLSA Status: Non-exempt, hourly

Key Responsibilities

1. Supervisory Responsibilities

  1. Observe daily nursing care provided by assigned personnel.
  2. Participate in counseling and disciplinary measures as appropriate.
  3. Ensure staff are properly oriented and assigned according to skills, licensing, and training.
  4. Assist in evaluating educational needs and work performance of nursing staff.
  5. Work in any nursing capacity when staffing shortages occur.
  6. Conduct regular team meetings in coordination with the AL Administrator.
  7. Collaborate openly with nursing coordinators, Director of Nursing, and Healthcare Administrator.

2. Infection Control

  1. Monitor compliance with infection control standards.
  2. Implement precautionary measures for residents displaying signs of infection.
  3. Ensure staff practice universal precautions and proper waste disposal.
  4. Maintain infection control documentation.

3. Coordination of Resident Care

  1. Oversee therapeutic programming and resident care quality.
  2. Participate in care plan conferences and evaluate care plans.
  3. Confer with residents and families to address concerns and resolve conflicts.
  4. Assess and report accidents, incidents, and medication errors.
  5. Monitor wound care and communicate updates appropriately.
  6. Ensure physician orders are obtained and implemented promptly.
  7. Assist with admissions, transfers, discharge planning, and pre-admission screenings.
  8. Monitor supply levels and review chart documentation.
  9. Coordinate lab work, x-rays, and physician appointments.
  10. Supervise medication and treatment administration.
  11. Ensure detailed shift reports are provided.
  12. Complete required federal and state assessments.
  13. Promote a positive image of The Wesley to residents and families.

Qualifications

  1. Current LPN or RN license in the State of Kansas
  2. Long-term care and/or management experience preferred
  3. Strong geriatric assessment skills
  4. Ability to read, write, speak, and understand English
  5. Ability to issue and follow oral and written instructions
  6. Strong interpersonal skills with residents and families
  7. Understanding of adult care home licensure regulations
  8. Ability to maintain effective working relationships across departments
